Limitations and evidence gaps
Most BPC-157 publications come from one interconnected investigator network, and related models or cohorts should not be counted as independent replication. The evidence is predominantly preclinical, often with small animal samples and limited reporting of masking, power, or multiplicity. Outside-network replication is narrow, and some musculoskeletal findings are mixed or null.
No completed randomized human study has been published in full. Animal histology, biomarkers, lesion area, and cell-migration findings do not establish human healing or clinical benefit, and differences in free base, acetate, impurities, aggregation, sterility, and endotoxin limit comparisons across test materials.
Published studies do not adequately address chronic toxicity, carcinogenicity, fertility, pre- or postnatal development, juvenile exposure, immunogenicity, aggregation, or delayed human adverse effects. FDA review also identified incomplete toxicology reporting and changes in coagulation, liver enzymes, glucose, triglycerides, hematology, and creatinine that require qualification rather than blanket safety claims.
Human pharmacokinetics, target pharmacology, and long-term safety remain unresolved. Route-specific impurities, sterility, endotoxin, residual solvents, and uncertain product form create safety questions separate from the pharmacology of a correctly identified peptide.
